What to include

  • Patient name, date of birth, phone number and insurance information
  • Diagnosis and current depressive symptom severity
  • Prior antidepressant trials with dose and duration
  • Current medications, including any oral antidepressant
  • Relevant cardiovascular or substance-use history
  • Your contact details for collaborative follow-up

What happens next

We contact the patient to schedule an evaluation and confirm eligibility.

We verify benefits and submit the prior authorization for Spravato.

Once treatment begins, you receive updates on response and plan changes so care stays coordinated with your practice.
